The Macon County Health Department (MCHD) is seeking a compassionate, organized, and collaborative professional to join our team as a HealthWorks of Illinois Case Manager.
This position plays an important role in helping ensure that children and youth in the legal care and custody of the Illinois Department of Children and Family Services (DCFS) have access to appropriate healthcare services and an established network of healthcare providers.
The HealthWorks program serves an eight-county region consisting of Clark, Cumberland, Coles, Edgar, Douglas, Macon, Moultrie, and Shelby counties.

What You’ll Do
As a HealthWorks Case Manager, you will help coordinate healthcare services and resources for youth in DCFS care. Responsibilities include:
- Conducting assessments and reassessments and developing service plans and referrals based on participant needs.
- Developing and maintaining a network of qualified healthcare providers serving youth in care.
- Building collaborative relationships with primary care providers, specialty providers, caregivers, DCFS staff, and medical case management agencies.
- Coordinating initial and specialty healthcare referrals.
- Providing interim medical case management services for youth newly entering DCFS care.
- Maintaining required case information and documentation in the Statewide Automated Child Welfare Information System (SACWIS).
- Assisting with the transfer and coordination of medical case management services based on program requirements.
- Providing information and resources to youth in care and their caregivers regarding available healthcare providers.
- Providing training and program updates to network providers, caregivers, HealthWorks Medical Case Managers, and DCFS staff.
- Helping identify and resolve healthcare access concerns, provider concerns, and other barriers through the program’s ombudsman services.
- Completing HealthWorks program audits and participating in applicable workshops, trainings, and conference calls.
- Maintaining confidential client information while providing professional and responsive customer service.
These responsibilities reflect the case management, provider-network, SACWIS, training, audit, and ombudsman functions identified in the current position description.
Minimum Qualifications
Candidates must meet one of the following:
- Bachelor’s degree in behavioral science, social science, or a health-related field; OR
- Bachelor’s degree in another field plus at least one year of experience in child, family, or community services; OR
- Associate degree plus at least two years of experience in child, family, or community services.
