Prairieland Service Coordination, Inc. Case Manager/QIDP Decatur, IL
We are looking to add to our Prairieland family in Decatur, IL. Prairieland is seeking a Case Manager/QIDP to serve Macon and Piatt counties. Bachelors degree in human service related field and one-year cumulative experience providing direct service to individuals with developmental disabilities required. The one-year cumulative experience requirement can be met through employment, volunteer or family care. Excellent benefits, schedule 8:00 am to 4:30 pm, Monday through Friday, with the flexibility to work partially from home.
Case Manager/QIDP – Job Responsibilities
Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc., the Case Manager/QIDP assesses individual needs and assures the provision of necessary services through communication, service coordination linkage with provider agencies, service monitoring, and individual advocacy. This role also includes maintaining accurate individual files, providing consultation to provider agencies, coordinating specialized services for eligible individuals, entering daily billings for payroll, participating in staff meetings and trainings, reporting service trends or issues to management, and performing other related duties as assigned.
Qualifications
- Bachelor’s degree in human services and QIDP eligibility per DHS requirements.
- Ability to organize, plan, and follow schedules on a daily, weekly, monthly, and annual basis.
- Knowledge of the developmental disabilities service system.
- Ability to work effectively with professionals, social service organizations, individuals, and families.
- Ability to work both independently and collaboratively with minimal direct supervision.
- Flexibility to work outside normal business hours when necessary.
- Receptive to direction, constructive feedback, and accountability.
- Professional communication and adherence to the Employee Code of Ethics.
- Reliable transportation, valid Illinois driver’s license, and auto insurance.
Physical Expectations
- Ability to read, write, and communicate effectively.
- Ability to ambulate, sit, bend, and carry files.
- Ability to lift up to 5 lbs. or pull a carrier.
